What Type of License Do You Need?

If you are currently (or have been) an Oregon nurse or nursing assistant, please review the application options below. You canrenew or reactivate your current or expired Oregon license/certificate, or reinstate licenses/certificates that have been revoked or voluntarily surrendered.

You hold a current license/certificate that is active or expired fewer than 30 days

​If your Oregon nursing license or nursing assistant certificate is active, or has been expired fewer than 30 days, you may apply to renew your license or certificate.

What happens next: Once we receive your completed application and payment, we will begin processing your application. You will receive an email from us once your license/certificate has been renewed.

You hold a license/certificate that has been expired 31 days or more

​If your Oregon nursing license or nursing assistant certificate has been expired for 31 days or more, you may apply to reactivate (please see applications for specific requirements) your license or certificate:

  • You are a RN or LPN who can meet the practice requirement.
  • You are an advanced practice nurse.​
  • CNAs:
    • You are a CNA who either completed a training program within the last two years, or you can meet the work requirement of having at least 400 paid work hours (doing CNA authorized duties under the supervision or monitoring of a RN) in the past two years,andyour certificate must have been expired, retired, or inactive prior to 12/31/2019. Applicants whose certificate was active at any time after 1/1/2020 are not eligible.
    • You are a CNAwho can meetthework requirement of having at least 400 paid work hours (doing CNA authorized duties under the supervision or monitoring of a RN) in the past two years, but your certification expired on or after 1/1/2020​, orwho is a student nurse, a nurse who has graduated within the last year, or licensed LPN/RN that is wanting to reactivate your expired CNA certification, see the application for details.
  • You are a CMA who meets the work requirement of having at least 400 paid work hours doing CMA authorized duties under the supervision or monitoring of a RN in the past two years, and has completed eight hours of CE in pain management before your certification expired. If you did not have 400 CMA work hours or did not complete the CE hours before your certification expired, you must retake and pass the CMA examination to reactivate
